What is the Community Care Program?
The Community Care Program (CCP) is a state-approved in-home support initiative designed to help older adults remain safely and comfortably in their own homes. Instead of moving into assisted living or a nursing facility, eligible individuals receive professional, non-medical support services—customized to their needs.
Eligibility Criteria
To qualify for our Community Care Program through Aria Home Care, you must meet the following requirements:
1.Age Requirement: Be at least 60 years old or older.
2.County of Residence: Reside in one of the following Illinois counties:
- Cook County
- DuPage County
- Lake County
- McHenry County
- Kane County
- Will County
Assessment of Need: Undergo an in-home assessment to determine the level of help required with daily living activities (like bathing, dressing, meal prep, mobility).
Financial / Asset Requirements: Eligibility typically includes considerations of income and assets (for example, countable assets below a certain threshold, excluding your primary residence and vehicle).
Willingness to Participate: Be willing to work with our team to develop a personalized care plan and allow for regular reviews to ensure the right level of support.
Services Covered Under the Program
Once approved, you may access a wide range of services through Aria Home Care’s Community Care Program:
- Personal care (bathing, grooming, dressing)
- Mobility assistance and transfer support
- Meal planning and preparation
- Medication reminders and basic medication management
- Light housekeeping, laundry, shopping/errands
- Socialization and companionship
- Care coordination and caregiver support
- Family-member caregiver options (in certain cases, a qualified family member may be paid as a caregiver per program rules)
